Guide to SHN Services
This guide introduces you to Shipston Home Nursing: Hospice Care at Home.
We hope to answer some initial questions you may have and reassure you that we are here to support you and your family throughout this difficult time.
Who we are
Shipston Home Nursing is a hospice at home charity providing nursing care to patients with terminal illnesses who would prefer to spend the end of their lives at home. We are a small, dedicated team supporting adults who live in Shipston on Stour, Wellesbourne, Kineton, Bidford-on-Avon and surrounding villages and who are registered with GP practices in these areas.
Shipston Home Nursing is a registered charity. Thanks to the generosity of our supporters, our care is provided free of charge.
What We Do
We provide end of life nursing care so that patients wishing to be cared for at home can have the best quality of life possible following the diagnosis of an illness that cannot be cured.
Our team of registered nurses and healthcare assistants (HCAs) can help with:
- Personal care such as washing and dressing, and toileting needs
- Symptom control and pain relief. Our nurses will work with your GP to assess and manage pain and administer prescribed medication as needed
- Respite such as night or day sits so your family can rest
- Practical support and assessment of equipment needs. We can’t do things like household cleaning or shopping, but we can try to find services that could help with this
- Emotional support for patients and families, including pre-bereavement and bereavement counselling
Exactly what support we provide and when will depend on what you would find most helpful and your clinical needs. We often find patients need more intensive support and personal care during the last few months of life, but we are here to provide guidance throughout.
We work in conjunction with GPs, district nurses and other agencies.
Continuous Support
We provide day and night-time care on any day of the week, depending on your needs.
Our nursing team are available every day including weekends. We have a 24-hour phone line for advice and a nurse is available to speak to you at any time of the day or night, if you need our help.
We also work with the NHS Rapid Response Team which provides an urgent advice and call out service every day of the week from 8pm to 8am.
