How to request out support
You can refer a patient to our team or request our support by:
- Calling our nursing team on 01608 664850 (Office Hours) or on 07920 480392 (Duty Nurse Coordinator)
- Emailing us at [email protected] to request a call from a member of our nursing team

We aim to meet new patients within 48 hours of receiving a referral to complete an initial assessment and to understand your care needs and wishes. Assessments are completed by a member of our Senior Nursing Team, all of whom are trained to take you and your families through the process with empathy and compassion. Once the assessment has been completed the level of our involvement can be tailored to your individual needs. Our aim is to give you an opportunity get to know us and access our support at a pace that you are comfortable with.

Who can refer patients to SHN?
Patient-led requests for support – known as self-referrals – can be made directly by you as the patient, your family or friends.
We also receive referrals from general practitioners (GPs), district/community nurses, hospitals, other hospices and community care homes.
The GP surgeries we currently cover are:
- Bidford Health Centre
- Fenny Compton Surgery (including the Shenington branch)
- Meon Medical Centre
- Hastings House Medical Centres, Wellesbourne and Kineton
- Sibford Surgery
- Shipston Medical Centre
- Vale of Red Horse Health Centres, Kineton and Tysoe
We also care for patients from other surgeries who live within our geographical area which includes Shipston-on-Stour, Kineton, Bidford-on-Avon and Wellesbourne and surrounding south Warwickshire villages.
