Patient Referrals

Our Nursing Care is completely free of charge. We look after patients who either live in our defined geographical area or who are registered with our partner GP Surgeries.

Patient referrals can be made to us in a number of ways:

• Self Referral – directly by the patient, their family or friends by calling us on 01608 664850 (Office Hours) or on 07920 480392 (Duty Nurse Coordinator)
• General Practitioners (GP’s)
• District/Community Nurses
• Hospitals/Hospices/Community Care Homes

We aim to meet any new patient referred to us within 48 hours to complete an initial assessment and to understand their care needs and wishes. Once the assessment has been completed the level of our involvement can be tailored to the needs of the individual. This creates an opportunity for our nurses to build a relationship with the patient and family at a pace that they are comfortable with.

All assessments are completed by a member of the Senior Nursing Team who is trained to gain the information from patients and families in a conversational way with empathy and compassion.
