We are here to help
If you or someone you care for is living with an incurable illness that no longer responds to treatment and you wish to be cared for in your own home, our hospice care at home service may be right for you.
Our support can begin from the point of diagnosis or referral to palliative care so that we can get to know you, your family and carers and help plan the care you might need – even if you don’t need nursing support for some time.
We understand this can be a difficult time and we are here to help you and your family both practically and emotionally. You can request our support directly (self-referral) or be referred by your GP or other health professional.
How we can support you
Our team of registered nurses and healthcare assistants (HCAs) can help with:
- Personal care such as washing and dressing, and toileting needs
- Symptom control and pain relief
- Respite such as night or day sits so your family can rest
- Practical support and assessment of equipment needs
Exactly what support we provide and when will depend on what you would find most helpful and your clinical needs. We often find patients need more intensive support and personal care during the last two months of life, but we are here to provide guidance throughout.
Our Team
Our nursing team is led by Head of Nursing Services, Anna Briggs. Anna is a registered nurse with experience in nursing and palliative care.
Our nursing coordinators are supported by a team of registered nurses who work directly with our patients. Our nurses can administer medication and manage symptoms. They are supported by our experienced healthcare assistants (HCAs) who are trained in personal care.
We usually work alongside GPs, district nurses and other agencies and all nursing activities are supervised by the Care Quality Commission (CQC).
When to contact us
We appreciate that contacting a hospice service can be daunting. However, getting to know you early helps us to understand your needs and wishes and gives us the opportunity to improve your quality of life.
